Updated on 04/15/2024

*Current Physical (4/15/18 or after)- please use Hometown Urgent Care if you need a physical. They take walk ins and are open 7 days a week.
*Paid $25 Insurance Fee to Athletic Office or online at Kalamazoopublicschools.com using RevTrak on the home page (once per school year)
*Signed Athletic Code of Conduct (once per school year)

Once all of the information is collected, a grade check will take place. Students must be passing 4/5 classes to be eligible. If students do not pass 4/5 at the end of the first trimester (November 29) they will be ineligible for the entire season.
Once the grade check has been passed, the coach will receive a blue card indicating the athlete is eligible to begin tryout/practice. If they do not have a blue card, they cannot tryout for a team.
